<p style="text-align:justify"><strong>INFORMATION FOR COMPLETING THE FORM</strong></p> <ul> <li style="text-align:justify"><strong>The following browsers are recommended: Mozilla Firefox, Google Chrome or Microsoft Edge. </strong></li> <li style="text-align:justify">fields marked with an asterisk (*) in the application form are compulsory;</li> <li style="text-align:justify">the sections that the user has completed will be highlighted in grey, those still to be completed in blue, and in the summary section, any sections not completed&nbsp;are marked in red;</li> <li style="text-align:justify">when filling in the form, do not use the &quot;back&quot; button in&nbsp;the browser; to modify a previously filled in and saved section, click on the title of the desired section and at the bottom of the page, select the button &quot;GO back to edit&quot;.</li> </ul> <p style="text-align:justify"><strong>The self-declarations requested in the course of the application are made in accordance with art. &nbsp;46 and 47 of DPR 445/2000. &nbsp;Self-declarations may be used by Italian and European Community citizens without restriction. For Non-EU-citizens legally residing in Italy, the provisions of the D.P.R. 445/2000, art. 3 apply, which allow the use of the self-declarations as per art. 46 and 47 of the D.P.R. 28th December 2000, no. 445 only if it is a question of proving states, facts and personal qualities that can be certified by Italian public bodies. </strong></p> <p style="text-align:justify">Untrue declarations are punishable under the Penal Code and special laws on the subject.</p> <p style="text-align:justify">Upon completion you have the following options to validate the document:</p> <ul> <li style="text-align:justify"><u>by <strong>handwritten signature</strong></u>: the user must manually sign the paper document (previously printed), scan it using a scanner or cell phone camera and upload it to the designated field in PDF or JPG format along with a front and back copy of a valid ID document in PDF or JPG formats. <strong>NB</strong>: Manual signature performed on electronic media (e.g., tablet) or <strong>an image of the signature copied on the document do not correspond</strong> to either a handwritten signature or a digital signature.<br /> &nbsp;</li> <li style="text-align:justify"><u>via <strong>digital signature of the document</strong></u>: the user will have to download the PDF document of the form and affix his digital signature as instructed by his signature service provider.
